Closing date 31 Mar 2026 Social Prescribing Link Worker – 37.5 hours (part time hours considered for right candidates) Would you like to work for an organisation that believes that everyone, regardless of the challenges they face, should have the opportunity to thrive? Involve Kent aims to improve health and empower people and communities to live well. We are looking for a Social Prescribing Link Worker to work in the West Kent area to provide personalised support for clients to improve their wellbeing. Taking a holistic approach, you will support and connect people to community groups and services to provide them with practical and emotional support. Funded by the NHS our pioneering prevention focused, and outcomes based Connect Well West Kent Social Prescribing Service develops outreach ‘social prescribing’ focusing on people facing health inequalities and those with long term health conditions. You will need to be able to drive and have access to a vehicle for this role, as it involves community and home visits. The role includes providing regulated activity with adults, so employment is subject to an Enhanced DBS check.
