Open Positions : Job title : Service Coordinator/QIPD House Name : Hamden, Connecticut Hours : 40, Full Time, Exempt P ay Starting at $55,000 Aspire Living & Learning is looking for a Service Coordinator/QIPD. The Service Coordinator/QIDP at Aspire Living & Learning plays a vital role in overseeing the delivery of active treatment services in ICF group homes for adults with developmental disabilities. The Service Coordinator/QIDP leads the development, coordination and monitoring of individualized program plans to ensure services promote independence, skill development and quality of life. Our Ideal Candidate: Bachelor’s degree in human services related fields such as psychology, social work, special education, rehabilitation counseling, sociology, or a related discipline At least 1-year direct experience working with individuals with intellectual or other developmental disabilities including experience supporting or implementing individualized programs Ability to analyze data, identify trends, and make informed program recommendations Demonstrated commitment to support individuals with dignity, respect and person-centered care with empathy, enthusiasm and passion. Excellent interpersonal, organizational and communication skills. Basic Computer skills, experience using Therap a plus!
