RN Case Manager – Homecare & Hospice Location: North Conway, NH Schedule: offering flexible schedules to support work-life balance. 24 to 40 hours a week. About the Role As an RN Case Manager, you’ll provide compassionate, highquality nursing care to patients in the comfort of their homes. You’ll guide patients and families through recovery, chronic illness management, or endoflife care, ensuring dignity, comfort, and exceptional clinical outcomes. This is a meaningful, relationshipdriven role where your clinical expertise and emotional intelligence truly matter.
