About the Role
As a Clinical Pharmacist with PharmD Live, you’ll engage directly with patients to deliver proactive, pharmacist-led clinical care that improves outcomes and drives value-based success. You’ll provide services such as Chronic Care Management (CCM) , Transitions of Care Management (TCM),
Remote Patient Monitoring (RPM) , and Medication Therapy Management (MTM) through secure telehealth platforms, ensuring each patient receives personalized, evidence-based support.
Key Responsibilities
• Deliver disease and medication management services via phone or telehealth (video).
• Conduct comprehensive medication reviews to identify therapy gaps and medication-related issues.
• Monitor drug therapy for contraindications, drug-drug or food interactions, and dosing accuracy.
• Recommend cost-effective, clinically appropriate alternatives to medical staff.
• Collaborate closely with internal care teams and external partners to coordinate patient care.
• Document all clinical and care management activities in PharmD Live’s proprietary system.
• Educate patients on medication use, adherence strategies, and treatment goals.
• Contribute to quality initiatives by sharing insights and supporting program enhancements.
• Maintain professional growth through ongoing education and research.
• Provide clinical oversight and support to ensure timely and value-driven client delivery.
• Present updates and progress reports to leadership as needed.
Qualifications & Skills
• PharmD from an ACPE-accredited College of Pharmacy.
• Active Minnesota pharmacist license or eligibility for licensure.
• Board Certification (BCGP, BCPS, BCACP) or willingness to obtain within 12 months.
• Demonstrated independence and sound clinical judgment.
• Current knowledge of all phases of pharmacy practice.
• Strong communication, documentation, and analytical skills.
• Ability to manage multiple priorities in a dynamic care environment.
• Proficiency with Google Workspace and Windows platforms.
• Empathetic, patient-centered, and collaborative approach to care delivery.
