Registered Occupational Therapist - Permanent full time - Community Health Centres of Northumberland Registered Occupational Therapist – Permanent full time RECEIVING APPLICATIONS FROM: February 27, 2023 - April 14, 2023 Description Registered Occupational Therapist – Permanent – Full Time The CHCN GAIN team is an interprofessional team of highly skilled geriatric clinicians that provide comprehensive geriatric assessments and specialized, goal-based care to older adults living with multiple, complex health conditions including frailty. Through evidence-informed and person-centred approaches, the team aims at helping clients with improving/maintaining their function & independence, thereby preventing hospitalizations and crises. Our services are provided at the clinic and in the community i.e., through home visits (where required). The CHCN GAIN team comprises of a Nurse Practitioner, Care of the Elderly (COE) Physician, Occupational Therapist, Social Worker, Pharmacist, Behavioral Support Nurse, Personal Support Workers, Nurse Care Coordinator, and a Clinical Admin. The team is also supported by consultant Geriatricians and Geri-psychiatrists. The CHCN GAIN team is part of a broader network of 12 GAIN teams throughout the Central East Region of Ontario. Position Summary The GAIN Occupational Therapist: • conducts triages, comprehensive geriatric assessments (including cognitive testing), and provides geriatric and discipline-specific expertise to support the development of collaborative care plans • conducts discipline-specific assessments (including in-home OT assessments) and implements interventions to promote safety, functional independence, and rehabilitative/restorative potential • supports clients and care partners with system navigation and following through with GAIN recommendations • participates in case discussions and team meetings • serves as a liaison between clients, care partners, and other service providers. Experience Qualifications/Experience MSc Occupational Therapy or equivalent required • Current Registration with the College of Occupational Therapists of Ontario required • Previous experience in clinical work with older adults (65 and older) • Experience working effectively as part of an interprofessional team • Training in P.I.E.C.E.S.™, U-First!™, or Montessori Methods for Dementia ™. Core Competencies Knowledge • Fundamental understanding of physiological and biopsychosocial mechanisms of the aging processes, geriatric syndromes, and the impact of frailty • Knowledge of the various domains of comprehensive geriatric assessment • Knowledge of different cognitive testing tools and functional assessment tools • Advanced understanding of dementia and the ability to assess the unique impact of the disease • Demonstrated knowledge of best practices for dementia, delirium, depression and other mental health issues, and their effect on clients, families and/or care partners • Knowledge of community resources. Clinical Skills • Works to full OT scope of practice • Demonstrated skill in working with older adults with significant functional deficits and communication challenges (e.g., cognitive impairment, sensory impairment, behavioral problems, neurological deficits, or ethno-cultural pluralities) • Demonstrated skills in conducting in-depth assessments to support care planning and effective clinical decision-making • Maintains complete, accurate and timely records to ensure continuity of client care • Employs cognitive assessment tools in a standardized and consistent manner. Collaboration and Communication • Ability to work independently and co-operatively in a busy interprofessional environment • Participates in quality improvement activities and is an active member on selected committees and/or working groups at CHC-N • Responds to inquiries in a timely fashion • Demonstrated computer skills including the ability to use clinical documentation software and other electronic communication tools Professional and Work Ethics • Adheres to the Mission, Vision, and Values of CHC-N • Ensures practices consistent with ethical standards outlined by the College of Occupational Therapists • Demonstrates respect for patient’s privacy and confidentiality • Proven good attendance record, organizing and prioritizing work effectively. Other Position Requirements • Fluency in English is required • Police clearance documentation will be required for employment • Must possess a valid driver’s license and have regular access to an insured vehicle as the position may require travel throughout Northumberland.
