Ambulatory Registered Nurse   (RN) – Rheumatology DEPARTMENT: Rheumatology – Baystate Medical Center Baystate Medical Center Campus 759 Chestnut Street Baystate Health is   seeking a compassionate   Registered   Nurse to join   our Rheumatology team . This   is an exciting   opportunity to make   a meaningful impact   by coordinating care   for patients with   chronic and complex   conditions while partnering   closely with physicians, advanced practice providers, and a   multidisciplinary care   team. As an Ambulatory   RN, you will   provide clinical triage, patient assessment, education, care coordination, and   ongoing follow-up. You'll manage patient   portal communications,   conduct nurse visits, support treatment plans, and serve   as a trusted   resource for patients   throughout their care   journey. The ideal   candidate is an   excellent communicator   who thrives in   a collaborative, patient-centered   environment and is   committed to delivering   exceptional ambulatory   care. SCHEDULE: • Full-Time • Monday–Friday |   8:00 AM–4:30 PM QUALIFICATIONS: • Current Massachusetts Registered   Nurse (RN) license. • Current AHA BLS certification. • Minimum of two   (2) years of   RN experience. • Strong assessment, triage, patient education, and   care coordination skills. • Excellent   verbal, written, and   interpersonal communication   skills. • Demonstrated ability to   collaborate effectively   with providers and   interdisciplinary teams. • Commitment to delivering   compassionate, patient-centered care to   individuals from diverse   backgrounds. • Ambulatory or specialty   practice nursing experience. • Experience caring for   patients with chronic   or complex medical   conditions. • Adult and pediatric   nursing experience, or   a willingness to   work with pediatric   patients.
