Description
Program in Assertive Community Treatment (PACT) is a multi-disciplinary, evidence-based outreach program that assists those with serious mental illness, integrate into the community. Bright Harbor Healthcare provides PACT services to more than 160 people in all of Ocean County. A Team consists of various specialties that may include, but not limited to: a Psychiatrist, Advanced Practice Nurse, RNs, Masters Level and Licensed Clinicians, Co-occurring Specialists, Vocational Specialists, MH specialists, and Peer mental health specialists. The Team shares in most responsibilities as a whole and maintains contact with individuals despite re hospitalization, incarceration or homelessness.
