Branding Hearts Home Health - Private-Duty Nurses - RNs and LVNs - Pediatric Home Health Private-Duty Nurses - RNs and LVNs - Pediatric Home Health Private-Duty Nurses - RNs and LVNs - Pediatric Home Health San Antonio Job Type Full-time, Part-time Description Job Summary: • Provides specialty care and services operating in a range of clinical settings. • Assess, monitor and document patient’s needs and care. • Collaborates with other health care providers to discuss patient care. Essential Functions: • Acts as liaison between patient and family and medical representatives. • Acts as liaison between patient and family and medical representatives. • Performs patient evaluations by monitoring vital signs, administering medication, placing IVs, etc. • Develops day-to-day care plans for patients in hospital and once they are on home-care programs. • Plans home-care health options for patients upon return home. • Obtains health history information and documents current physical conditions. • Practices most current safety and health code standards • Supervises staffed licensed vocational nurses, PAS/PCS staff, as needed • Able to lift 25 lbs • Frequent standing, walking, sitting, stooping, kneeling or crouching • Work independently and with team • Knowledge of relevant drugs, medications, Current procedures, legal and ethical standards. • Able to work independently without continual supervision • Able to work with stressful environments, people, or situations. Requirements Job Qualifications: Educations: Degree as a Registered Nurse Licensure: Current Texas State License as an RN, current Texas Driver’s License. Experience: Minimum 6 months’ experience or one year as a RN. Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Professional demeanor and appearance. Knowledge of relevant drugs, medications. Excellent oral and written skills. Demonstrate organizational and leadership skills. Transportation: Reliable transportation and valid and current auto liability insurance. Environmental and Working Conditions: Works in a patient’s home in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather. Physical and Mental Efforts: Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient/family individualized psycho social needs.
