Our team is the heart of it all: mission-driven, values-oriented, and relentlessly committed to taking the best care of our patients.   About the Role Primary Care Physicians provide daily care to patient panels capped at approximately 500 patients. This model emphasizes work-life balance while delivering a concierge-like experience for patients. With a panel size limited to 500, physicians typically see around 12 patients per full day, allowing 30 to 45 minutes per visit to address each patient's needs thoroughly. Patient appointments are generally scheduled between 9:00 a.m. and 3:30 p.m., leaving additional time for urgent patient needs and administrative tasks. Administrative burdens are significantly reduced thanks to a dedicated team of coders who handle pre-charting and post-visit reviews, streamlining clinical documentation. Medical assistants manage administrative and care coordination tasks, enabling physicians to focus entirely on patient care and providing an exceptional experience.   Office Hours: Monday-Friday 8am-5pm   Key Responsibilities Primary Care Physicians will provide comprehensive, patient-centered care to geriatric patients within a managed care, value-based care model, ensuring optimal health outcomes and a high-quality care experience. Deliver Patient-Centered Care Perform thorough assessments and develop personalized care plans for geriatric patients. Manage chronic conditions, preventive care, and acute issues within the scope of value-based care principles. Utilize evidence-based guidelines to optimize patient outcomes and quality metrics. Coordinate Care Across the Healthcare Team Collaborate with your team and specialists to ensure seamless care delivery. Lead care plan discussions and participate in team meetings to review patient progress. Focus on Value-Based Outcomes Achieve quality metrics by proactively addressing care gaps, reducing unnecessary hospitalizations, and improving patient engagement. Conduct routine follow-ups and monitor patient progress to ensure adherence to care plans. Utilize Health Technology and Data Leverage electronic medical records (Epic or eCW) to document care, track patient outcomes, and support care coordination. Utilize data analytics to identify trends and opportunities for improving patient care and practice efficiency. Provide Exceptional Patient Experience Build trusting relationships with patients and families, addressing their unique needs and concerns.
