As a Primary Care Provider , you will provide primary care to patients in long-term care facilities. This includes: • Developing and implementing personalized treatment plans based on patient needs • Offering medication management, including prescribing and monitoring medications • Collaborating with a multidisciplinary team of healthcare professionals to ensure patient care • Maintaining accurate medical documentation in line with regulatory standards • Providing education and support to patients and their families regarding mental health conditions and treatments Qualifications: • Current Nurse Practitioner license in specified location • At least two years of experience is required • Experience providing care to geriatric populations in long-term care settings is preferred but not required • Excellent communication skills, especially when working with facility staff and residents' families • Ability to manage time effectively, including flexibility in scheduling and being responsive to patient needs • Ability to manage time effectively, including flexibility in scheduling and being responsive to patient needs MDB Health Services Overview For more than 13 years, MDB Health Services has provided medical and psychiatric care to residents of long-term care communities across Mississippi, Louisiana, Arkansas, Tennessee, Kentucky, and Texas.
