Position Details:
• Main Office location: Westwood, MA
• Service Area: Quincy, Westwood, Brockton, Scituate, Attleboro areas.
• Job Type: Per Diem
Responsibilities:
· Conducting psychosocial assessments related to the patient's illness and environment and effectively communicating findings to the registered nurse.
· Performing social evaluations and developing appropriate intervention plans based on assessment findings.
· Maintaining accurate and up-to-date clinical records for all patients referred to social work.
· Providing essential information and referral services to patients, families, and caregivers regarding practical and environmental needs.
· Acting as a liaison between patients, families, caregivers, and community agencies.
· Building and maintaining collaborative relationships with other healthcare professionals to support comprehensive patient care.
· Working with public and private agencies to leverage available resources for the benefit of our patients and our organization.
· Participating in the development of comprehensive care plans and engaging in case conferences as required.
· Assisting physicians and the multidisciplinary team in understanding the social and emotional factors influencing health problems.
· Contributing to discharge planning and ensuring a smooth transition for patients and their families.
Qualifications:
· LICSW (Licensed Independent Clinical Social Worker) required.
· Sensitivity and understanding of end-of-life issues.
· Expertise in psychosocial assessment and treatment.
· A collaborative approach to working with a multidisciplinary team.
· At least one (1) year of experience in Hospice care preferred.
· Strong verbal and written communication skills, as well as good organizational skills.
· Maintain current BLS CPR certification.
· Possess a valid driver's license and own a reliable, insured vehicle.
Benefits & Compensation:
· Rates: $60 per visit + mileage reimbursement.
