Reports to: Clinical Manager, Supportive Care
Salary: $43.47 - $54.34 per hour (To commensurate with experience and consistent with UHN compensation policy)
Hours: 14 hours/week
Status: Part-Time Temporary contract for 1 year
Shifts: Monday - Friday, 9:00am-5:00pm (2 days/week)

As an integral member of the Cancer Rehab and Survivorship Program (CRS) and a Regulated Health Professional, this Occupational Therapist will be responsible for the delivery of clinical programming and education related to rehabilitation from treatment for cancer. Primary involvement will be completing comprehensive assessments of new patients and providing follow up appointments based on patient needs.
Other duties include: using health coaching skills to improve adherence to patient goals; facilitating virtual lymphedema classes; monitoring/stocking/ordering of lymphedema supplies; discharge planning for patients upon program completion; liaising with community partners and updating community CDT and ADP lists; triaging referrals; completing documentation in a timely manner; quality assurance, administrative, and program related marketing activities; support with clinical research activities; teaching of learners/observers; liaising with members of the interdisciplinary team; and other duties as assigned.
