Services to be Performed :- Assess and develop interventions for mitigation of fall risks and adaptive equipment including but not limited to the following: · Assess functional mobility to determine needs for mobility equipment · Notify FSH of equipment needed · Provide consumer/caregiver education on appropriate use and safety with adaptive equipment · Other interventions to mitigate fall risks as recommended Provide additional OT assessments and interventions including but not limited to those related to: • Activities of daily living/self-care • Fine motor coordination • Generalized weakness • Muscle re-education • Pain management Up to 24 hours per week. Hours will be worked between 8:00 am and 4:30 pm Monday through Friday.
