As an Occupational Therapist with Premier Home Health Providers, you will help patients regain independence, improve function, and safely navigate daily life — all from the comfort of their own home. Please note that this role is per diem with a rate of $110 - $120 per visit. Responsibilities Evaluate patients’ functional abilities and home environments to identify needs and safety concerns Develop personalized treatment plans and communicate them to physicians and the care team Teach and retrain patients in daily living skills affected by illness or injury Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices Maintain accurate clinical records and promptly report changes in patient condition Coordinate with the broader care team to ensure seamless, patient-centered care Educate family members and caregivers on the patient’s treatment approach Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service Support discharge planning as patients transition out of home health care Perform other duties as required for patient services
