Description
This position is located in the Purchased/Referred Care (PRC) department at Northern Cheyenne Service Unit, Lame Deer, Montana. Responsibilities include providing clinical case management for the PRC Program by coordinating healthcare services among inpatient and outpatient care teams, external healthcare providers, and the PRC Program.
