Mid-Level Provider (Family Nurse Practitioner or Physician Assistant) Location: Peterstown Health Center (Primary Location) Organization: Monroe Health Center Monroe Health Center is seeking a compassionate and dedicated Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our team. As a Federally Qualified Health Center (FQHC), we are committed to providing high-quality, patient-centered primary care to individuals and families of all ages, regardless of their ability to pay. The selected provider's primary practice location will be Peterstown Health Center . This position will also provide Provider coverage at other MHC Health Center locations as needed. Responsibilities • Provide comprehensive primary care services to patients of all age groups. • Diagnose and treat acute and chronic medical conditions. • Perform preventive health screenings and patient education. • Collaborate with physicians and other healthcare professionals to coordinate patient care. • Maintain accurate and timely medical documentation in the electronic health record. • Participate in an on-call rotation shared among providers. Qualifications • Current licensure as a Family Nurse Practitioner (FNP) or Physician Assistant (PA) in West Virginia (or eligible for licensure). • Current DEA registration (or ability to obtain). • Preferred: Minimum of two (2) years of family practice or primary care experience. • Commitment to providing compassionate, high-quality care to diverse patient populations.
