Job Summary Summit Medical Consultants is seeking a highly organized and compassionate Medical Assistant to support our Chronic Care Management (CCM) program. This role will focus primarily on coordinating care for patients with chronic conditions, improving patient engagement, and supporting providers in delivering high-quality, proactive care. This position is ideal for someone who enjoys building relationships with patients, coordinating care across disciplines, and making a meaningful impact on patient health outcomes. Responsibilities Schedule:
Monday-Friday, 8:00 AM-5:00 PM. This position includes a flexible mix of facility visits, work in the office, and work from home, based on the needs of the business. Chronic Care Management Conduct monthly outreach visits and follow-up calls with CCM-enrolled patients and families Assist with patient enrollment and education regarding CCM services Coordinate care plans and ensure appropriate documentation within the electronic medical record Monitor patient needs and communicate concerns to providers and care teams Support medication reconciliation and care coordination activities Assist with transitions of care and facilitate communication between providers, specialists, facilities, and community resources Maintain accurate documentation to support CCM compliance and billing requirements Administrative Support Manage practice fax communications and inbound phone calls Assist with scheduling and coordination of telemedicine visits Perform scanning, data entry, and general clerical duties Support providers and administrative teams with special projects and operational needs Complete additional administrative responsibilities as assigned Qualifications Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) preferred Minimum of one year of Medical Assistant experience Experience using Electronic Medical Records (EMR) Excellent communication, organization, and time management skills Strong attention to detail and ability to manage multiple priorities Ability to work independently and collaboratively in a fast-paced environment Reliable transportation required Preferred Experience Experience with Chronic Care Management (CCM), Care Coordination, Population Health, or Case Management Experience working with elderly or medically complex patient populations Experience with Google Workspace, Microsoft Excel, Word, and other office applications Experience working in a hybrid environment Prior experience supporting telemedicine workflows Benefits Locally Owned and Operated Competitive Compensation Medical, Dental, and Vision Insurance Paid Time Off Paid Holidays 401(k) Retirement Plan Opportunities for Growth and Professional Development Pay:
