Description BMS Family Health Center is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our behavioral health team. This role provides direct clinical services to adolescent and adult patients our primary care and school-based clinic settings. The LCSW will deliver high-quality, patient-centered care through individual, family, and group therapy, while ensuring all documentation meets established mental health standards. This role also contributes to a collaborative, interdisciplinary care environment. Fluency in English and Spanish is a plus! ESSENTIAL FUNCTIONS • Collaborate with the Director of Behavioral Health to support clinical goals related to adolescent and adult mental health services. • Conduct psycho-social assessments and patient needs analysis to develop treatment plans and appropriate referrals (e.g., psychiatric evaluation, hospitalization, in-home services). • Partner with clinical staff to implement and complete screenings for depression, anxiety, substance use, alcohol, and tobacco. • Provide individual, family, and group psychotherapy based on comprehensive assessments and treatment plans. • Coordinate therapy sessions for patients and families. • Deliver crisis counseling and intervention services as needed. • Communicate and coordinate care with ACS, case managers, and other collateral contacts. • Complete psychosocial assessments and develop individualized treatment plans, updating them regularly. • Maintain accurate and timely documentation in the Electronic Health Record (EHR), including closing progress notes within 48 hours. • Share relevant information with care partners to support optimal patient outcomes. Requirements REQUIRED QUALIFICATIONS • Master’s degree in social work from an accredited program • Valid New York State LCSW license • 5 or more years of social work experience • Bilingual in English and Spanish (advanced or native Spanish language speaker), preferred. • Completion of Mandated Reporter Training • Current CPR and Infection Control certifications • Strong organizational, time management, and planning skills • Excellent written and verbal communication skills. • Strong knowledge of child and adolescent development. • Ability to build positive, trusting relationships with youth and families. • Excellent clinical documentation and professional writing skills. • Proficiency with Microsoft Office Suite and Electronic Health Records. • Effective communication, active listening, and interpersonal skills. • Ability to identify high-risk psychosocial factors affecting patient health. • Demonstrated ability to work effectively in a fast-paced, interdisciplinary team environment. • Sound judgment and ability to make timely decisions in high-pressure situations. • Commitment to maintain confidentiality and ethical standards. • Participation in recredentialing and annual training requirements. Salary: $88,000 - 98,000 annually
