Key Responsibilities
The Home Health Licensed Practical/Vocational Nurse (LPN/LVN) is responsible for delivering direct patient care under the supervision of a Registered Nurse. This role involves implementing the established plan of care, administering treatments and medications as prescribed, and supporting patients and families. The LPN/LVN collaborates closely with interdisciplinary team members to promote comfort, dignity, and quality of life, and to ensure that patient care outcomes are met in accordance with home health standards and regulatory guidelines.
In this role you will be responsible for:
Provides direct patient care as defined in the State of Nursing Practice Act
Implements plan of care indicated by the clinician
Provides accurate and timely documentation consistent with the plan of care
Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care
Participates in coordination of home care services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, SLP) to the case manager or Clinical Supervisor
Uses equipment and supplies effectively and efficiently
Participates in personal and professional growth and development
Performs other duties as assigned by the case manager or supervisor
Required Qualifications
Graduate of an accredited practical nurse or vocational nursing program
Current state license as an LPN/LVN
1+ years experience as a nurse in a clinical care setting
Demonstrates the ability work collaboratively as part of an interdisciplinary team
Excellent verbal and written communication skills
Strong organizational and time management abilities
Solid clinical judgement and the ability to follow care plans accurately
Calm, compassionate demeanor
Reliable transportation and valid automobile insurance coverage
Desired Qualifications
Experience in home health or hospice preferred

8:00 am – 5:00 pm, Monday through Friday
