Registered Nurse (RN) – Hospice Care Full-Time | Fannin & Grayson Counties, TX Are you a compassionate Registered Nurse who believes every patient deserves dignity, comfort, and exceptional care at the end of life? Join our growing hospice team and make a meaningful difference in the lives of patients and families throughout Fannin and Grayson Counties. Position Summary As a Hospice RN, you will provide skilled nursing care, symptom management, patient and family education, and support to hospice patients in their homes, assisted living communities, and skilled nursing facilities. You will work collaboratively with an interdisciplinary team to ensure patients receive the highest quality end-of-life care. Essential Responsibilities • Complete comprehensive patient assessments and develop individualized plans of care • Monitor patient condition and manage symptoms in accordance with physician orders and hospice standards • Administer medications and treatments as prescribed • Provide education and support to patients, caregivers, and family members • Collaborate with physicians, social workers, chaplains, aides, and other team members • Maintain accurate and timely clinical documentation • Participate in on-call rotation as assigned • Ensure compliance with all hospice regulations, policies, and quality standards Qualifications • Current Texas Registered Nurse (RN) license • Minimum of one year of nursing experience required • Hospice, home health, or geriatric nursing experience strongly preferred • Strong assessment, communication, and critical thinking skills • Valid driver’s license, reliable transportation, and current auto insurance • Ability to travel throughout Fannin and Grayson Counties What We Offer • Competitive salary • Phone and Auto Allowance • Medical, dental, and vision benefits • Paid time off and holidays • Supportive team environment • Ongoing education and professional development opportunities • Opportunity to make a lasting impact on patients and families Why Hospice?
