As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.  This role is perfect for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships. This position will have you building report with patients, families, physicians, facilities, and other interdisciplinary team members. You'll have the autonomy to coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care. Location   Office Location: Jackson, MI Main Service Area Options:   Clinton / Eaton / Ingham Counties Jackson / Washtenaw Counties Hillsdale / Lenawee / Monroe Counties This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes Hours   Full Time: Monday to Friday, 8 a.m. to 5 p.m., with a weekend on-call rotation Part Time: Three weekdays, 8 a.m. to 5 p.m., with a weekend on-call rotation Per Diem: Flexible weekdays, Monday to Friday, 8 a.m. to 5 p.m., with a weekend on-call rotation Required Qualification Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Current State License as a Registered Nurse Minimum one (1) year experience as a nurse in a clinical care setting Reliable transportation, valid driver’s license, and valid automobile insurance coverage Preferred Qualifications Bachelor of Science in Nursing (BSN) Hospice or home health experience Essential Skills and Competencies Nursing skills as defined by accepted nursing standards Demonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment Demonstrates skill in nursin
