Heritage Hospice is excited to resume services in the Albuquerque community. Our dedicated team is back and ready to provide compassionate, high‑quality end‑of‑life care for patients and families across the region. About the Role We are seeking a dynamic, relationship‑driven Marketing & Education Liaison to represent our hospice program within the community and across healthcare partner networks. This role focuses on strengthening referral relationships, increasing awareness of hospice services, and providing education that supports appropriate and timely access to end‑of‑life care. The ideal candidate is an excellent communicator, comfortable engaging with clinical and community audiences, and passionate about helping patients and families understand the benefits of hospice care. Key Responsibilities Build and maintain strong partnerships with hospitals, physicians, SNFs, assisted living communities, home health agencies, and other referral sources. Deliver clear, compassionate education about hospice services, eligibility, philosophy, and referral processes. Conduct on‑site and community‑based presentations, in‑services, and outreach events. Serve as a consultative resource for healthcare professionals regarding hospice appropriateness and care transitions. Collaborate closely with intake, clinical leadership, and business development to support timely referrals and smooth patient admissions. Identify barriers to hospice utilization and develop targeted educational strategies. Track and document referral activities, contacts, and follow‑ups using CRM or organizational tools. Represent the organization at community events, conferences, and professional gatherings. Stay current on hospice regulations, Medicare benefit expectations, and industry trends. Qualifications Experience in hospice, home health, palliative care, senior living, or healthcare outreach strongly preferred. Exceptional communication, presentation, and interpersonal skills. Ability to explain clinical concepts clearly to both medical professionals and community audiences. Organized, proactive, and comfortable working independently in the field. Reliable transportation and valid driver’s license. Knowledge of hospice philosophy and Medicare guidelines preferred, though training is provided.
