Position Details: • Main Office location: Concord, MA • Service Area: Concord, MA and Surrounding Areas • Job Type: Per Diem • Schedule: Flexible • Hourly Rate: $25.00 Responsibilities: • Provide personal care assistance such as bathing, grooming, dressing, and mobility support • Observe and report changes in patients’ condition to the supervising nurse or therapist • Document care provided and patient observations accurately • Follow the care plan established by the Registered Nurse • Maintain a safe and supportive environment for patients in their homes • Promote quality, continuity, and safety of patient care Qualifications: • Certified Nursing Assistant (CNA) in Massachusetts preferred; Home Health Aide certification accepted • Ability to read and follow written care plans and document services provided • Ability to work independently with minimal supervision • Compassion and empathy for individuals who are ill, injured, or elderly • Current CPR certification • Strong communication and interpersonal skills • Valid driver’s license and reliable, insured vehicle Pathways Healthcare is committed to providing exceptional care to our patients and fostering a positive work environment for our team members.
