Administer medications and treatments as prescribed by physicians. Provide education and support to patients and their families regarding care plans. Document patient progress and report any changes to the healthcare team. Collaborate with interdisciplinary teams to optimize patient care. Ensure compliance with all safety and health regulations. Assist with personal care tasks, including bathing, dressing, and mobility support. Maintain accurate and timely records of patient care activities. Requirements: Current LPN license in the state of Maryland. Minimum of 1 year of nursing experience, preferably in home care or similar settings. Strong assessment and clinical skills with attention to detail. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Compassionate demeanor and a passion for patient-centered care. Basic Life Support (BLS) certification is preferred. Reliable transportation and willingness to travel to patient homes.
