Role Description The Bexley Health & Wellbeing Coaching Service is delivered through a partnership of voluntary and community organisations working together to improve health and wellbeing outcomes across the borough. The partnership combines specialist expertise, strong local connections and a shared commitment to helping people live healthier, more independent lives. Although Health & Wellbeing Coaches will be employed by individual partner organisations, they will work as part of a borough-wide coaching team. Coaches will collaborate closely with GP practices, Integrated Neighbourhood Teams, community organisations and local residents to tackle health inequalities and strengthen preventative support throughout Bexley. As a Vital 5 Health & Wellbeing Coach, you will empower residents to make positive lifestyle changes by focusing on the five key factors that have the greatest impact on long-term health and wellbeing: • Healthy Blood Pressure • Healthy Weight • Stopping Smoking • Healthy Mind • Drinking Less Working predominantly within North Bexley, the coach will take a proactive, prevention-focused approach, carrying out Vital 5 checks, supporting behaviour change, and connecting residents to local health and community services.
