Location: Cornwall You will lead a number of key work areas to support this work, including: • Realise the potential of physical activity and movement as a tool to transform patient outcomes in the prevention, treatment and recovery of long-term health conditions, developing opportunities for physical activity to contribute to local ICS priorities/plans and the NHS Forward Plan • Integrate physical activity and movement into health and care systems/pathways locally with a focus on support for delivery of Anticipatory Care, Personalised Care (health coaching, social prescribing and supported self-managed care) and Population Health Management • Empower health and care professionals (HCP), building capacity and advocacy for physical activity and movement by addressing the key barriers in enabling the workforce to engage patients/clients in brief interventions regarding physical activity behaviour change • Support the ongoing integration and transformation of health, care and VCSE (voluntary, community and social enterprise) sectors, using physical activity and movement as the conduit for connecting more of the system to itself with a focus on targeted place-based models of care To arrange an informal discussion about this opportunity, please email us at [email protected] and we will arrange for someone to call you back.
