What you will do: • Attend daily referral meetings and contribute to multi-disciplinary reviews, ensuring the best outcomes for service users. • Conduct comprehensive assessments of need, manage risks, and develop recovery plans tailored to individual clients. • Manage a caseload effectively, offering both one-on-one sessions and facilitating group interventions. • Collaborate with various services such as housing, employment, healthcare, and more to provide holistic support. • Promote and signpost services to ensure comprehensive care for service users. • Maintain accurate and professional records, adhering to data protection and GDPR requirements. • Support clients as they transition from the detention centre back to the community, preparing them for release or deportation. • Assist in service improvement initiatives and meet key performance indicators set by the Health and Wellbeing Manager.
