Position Summary On a flexible, as-needed basis, provides clinical services to all residents residing at Communities of Christian Living Communities (CLC). These services will include skilled nursing, assisted living, and/or rehabilitation services. Rehabilitation Flex Team Members are exempt from a multiple-shift requirement, and are encouraged to cover needs at multiple sites as they are able. Diagnoses and treats individuals with speech, language, voice, and fluency needs. This role is grounded in building genuine, trust-based relationships with residents and their families - we believe every person, team member and resident alike, brings unique strengths and perspectives that make our community stronger. Essential Duties • Performs assessments and diagnoses, and develops treatment plans for language and swallowing disorders. • Evaluates levels of swallowing, speech, and language difficulty and identifies appropriate treatment options. • Creates and carries out individualized treatment plans that address each resident's specific functional needs and personal goals. • Teaches residents techniques to improve sound production, voice, and fluency, tailored to what resonates with each individual. • Creates goals and objectives for residents to work toward, celebrating progress and working collaboratively through challenges. • Provides support and advice to residents, their caregivers, and family members with warmth and patience. • Counsels residents and families on ways to cope with swallowing and communication disorders, fostering trust throughout the process. • Builds strong relationships with residents over time, taking a genuine interest in their personalities, preferences, and everyday lives. • Compiles thorough notes on residents’ progress and adjusts therapies and treatment plans where necessary. • Documents resident care services according to approved medical and facility documentation procedures. • Collaborates closely with fellow team members, contributing to a supportive, inclusive workplace culture where diverse backgrounds and perspectives are welcomed and celebrated.
