Job Qualifications
Education : A Field Supervisor must be a licensed nurse or have completed two years of full-time study at an accredited college or university. An individual with a high school diploma or GED may substitute one year of full-time employment in a supervisory capacity in a health care facility, agency, or community-based agency for each required year of college
Experience : A minimum of one-year successful experience in supervisory role in home health care, hospice, or related health care service administration.
Skills : Excellent organizational skills and ability to effectively handle multiple tasks required.
Professional customer service skills and time management skills required.
Transportation: Reliable transportation. Valid and current auto liability insurance.
Environmental/Working Conditions: Performs duties during Agency operating hours. May require work in a routine office environment; noise level may be moderately high; must have the ability to work a flexible schedule and extended hours. Possible exposure to toxic materials, toner, etc. The ability to travel locally on occasion, some exposure to unpleasant weather.

Utilizing reports to monitor productivity and compliance of team members
Responsible for accurate and timely preparation and processing of HHS and department required documents; coordinates care of clients and utilizes task planners appropriately in Outlook. Ensures timeliness and completeness of all documentation.
Responsible for education to providers in client’s homes to perform HHS assigned tasks and timely preparation of documentation (timesheets.)
Establishes and maintains rapport with HHS case managers; assist with audits as required. Participates in and attends program IDT meetings as requested by HHS or Administrator. Participates in community relations activities as scheduled.
Adheres to HHS and MCO program requirements.
Available for on call rotations as required for all waiver programs.
Provides personal client assistance services in event provider staff are not available.
Performs supervisory attendant orientation visits to client homes for clients.
Maintains compliance with all At Home Healthcare policies, procedures, and regulations, including all federal and state laws.
Makes copies of various documents as required.
Maintains a clean and safe work environment.
Completes accurate and timely documentation.
Accurately prepares and proofreads letters, memos, spreadsheets and other correspondence.
