ENROLLMENT SPECIALIST (Bilingual) Who We Are Vigilance Health partners with FQHCs, ACOs, IPAs, and payers to deliver Population Health and Care Management services that actually move the needle. We run Care Management, APCM, Remote Patient Monitoring (RPM), and Behavioral Health Integration (BHI) programs — and we measure success the way our partners do: quality metrics, patient engagement, and outcomes that show up in data. Our purpose is simple, and we mean it: Change People's Lives — no matter their circumstances. The Role As our Enrollment Specialist, you'll be the face of our care management programs. This is a critical role that sets the tone for all of our future interactions with the patients. We are coaches to our patients - motivating and inspiring them to take action to manage their health. Some of the Enrollment Specialist’s roles include, but are not limited to: • Conducting telephone outreach to English and Spanish-speaking patients • Providing a clear and simple explanation of the benefits of our population health service lines • Quickly building rapport and trust with the patient to encourage them to enroll in our programs • Documenting call outreach and results accurately and concisely • Conducting eligibility verification, as needed Requirements Fluency in both English and Spanish is REQUIRED. At least 1 year of experience working in a high volume call center At least 1 year of experience in a healthcare setting preferred At least 1 year of experience in a consultative sales or customer-facing role preferred Must be proficient with Microsoft (Outlook, Word, Excel) Familiarity with Electronic Health Records preferred Must have excellent communication skills (verbal and written) and superb listening skills Reliable internet/ Wi-Fi connection
