What You ll Do
• Provide comprehensive outpatient family medicine care for adult patients
• Manage chronic conditions such as diabetes, hypertension, and hyperlipidemia
• Conduct preventive health assessments and screenings
• Collaborate with a multidisciplinary team to optimize patient outcomes
• Utilize EMR (EPIC) for documentation and patient management
• Participate in quality improvement initiatives
• Engage in community health promotion and education
What You Need
Required:
• Medical license valid in Colorado
• Board Certified or Board Eligible in Family Medicine
• Minimum 1 year of relevant clinical experience
• Strong interpersonal and communication skills
Preferred:
• Experience in outpatient primary care
• Ability to work collaboratively within a team
• Interest in community health
