Description
Outsourcing Advantage is seeking a passionate and organized Coordinator for Home and Community-Based Services (HCBS) . This role involves assisting individuals in various aspects of their care, maintaining up-to-date records, and ensuring a smooth onboarding process. The Coordinator will also foster collaborative relationships with clients' circles of support, handle office protocols professionally, and maintain high standards of internal and external communication. Responsibilities Develop and maintain personalized service plans for clients based on their unique needs and preferences. Maintain consistent check-ins with parents and Direct Support Professionals & .various community resources, healthcare providers, and support staff to ensure effective service delivery. Provide compassionate, competent, and trustworthy care Assist individuals in gift selection and event participation as needed Oversee a smooth and efficient onboarding process for new individuals. Support individuals in recruiting Direct Support Professionals (DSPs). Respond to emails and calls within 8 business hours. Ensure all individuals' forms are current and accurately maintained. Ensure compliance with HCBS regulations and best practices to maintain high standards of care and support. Assist in training and supervising direct support staff to promote effective service provision. Ensure timesheets and billing are completed accurately and timely. Provide support to DSPs and contract clients with missed clock-ins, thereby adhering to protocol. Requirements Strong verbal and written communication skills in English & Spanish. at least at a level of C2 Excellent organizational and multitasking skills, with the ability to prioritize client needs effectively. Empathetic and caring attitude towards individuals with diverse backgrounds and needs. Proficient in computer software for documentation, record-keeping, and communication (e.g., Microsoft Office Suite and client management systems).
