Benefits: 401(k) Competitive salary Flexible schedule Job Title- Community Liaison – Home Health Job description: We are seeking a dynamic and results-driven Community Liaison for Home Health to serve as a vital bridge between healthcare providers, patients, families, and community organizations. In this role, you will actively promote home health services, build strong referral relationships, and expand access to care within the community. This is a relationship-focused position requiring an individual who is already well-connected within the healthcare community and capable of generating and maintaining referral sources. Responsibilities Develop and implement outreach strategies to increase awareness and referrals for home health services through community engagement and provider relationships Build, maintain, and grow relationships with physicians, discharge planners, case managers, social workers, and community organizations Serve as a representative of the agency at meetings, health fairs, and professional events Identify and develop new referral sources while maintaining existing accounts Coordinate with internal teams to ensure smooth intake and patient transition into services Support program development efforts aligned with community needs Track outreach activities and referral trends to support growth strategies Manage social media and marketing initiatives to increase visibility Provide regular updates on referral activity, relationship status, and market opportunities Qualifications Established network of referral sources within the local healthcare community (required) (physicians, hospitals, SNFs, case managers, etc.) Proven experience in home health, hospice, or healthcare marketing/outreach Strong relationship-building and communication skills Ability to generate referrals and grow business through existing connections Experience with community engagement, public speaking, and presentations Strong organizational and follow-up skills Self-motivated with the ability to work independently and drive results What We’re Looking For Someone who can hit the ground running with an existing network Strong closer—comfortable building and maintaining referral relationships Not just outreach— someone who can convert relationships into admissions Professional, polished, and confident in clinical environments Why Join Us Growing agency with strong clinical support Opportunity to expand your network and influence in the community Competitive compensation with growth potential Ability to make a meaningful impact on patient care
