Position Overview Reporting to the Manager, Integrated Care & Experience, the Community Health Worker works in partnership with community organizations and service partners/providers as well as community groups and individuals to achieve delivery of health and wellness programs for the community. In this role, the Community Health Worker promotes and assists in the development, implementation and supervision of health and wellness programs. The Community Health Worker practices the principles of client-centered services within the context of a self-directed and inter-professional team approach. Additionally, this role will collaborate within an inter-professional team of health professionals across the community network that influence the health environment targeted to clients, as well as within the Agency’s team structure, in support of goals that align with and achieve the Agency’s strategy, Mission, Vision and Values. Key Responsibilities: Full supervision of two major programs - After school program and Canada Summer Jobs for summer camps Develops, implements and evaluates programs, services and community initiatives in response to community needs in order to achieve organizational objectives. Works with families (or individuals) in the community to identify their interests and potential and to help them develop meaningful activities, in line with program/services mandates. Ensures all necessary paperwork (e.g., registration forms, permission forms, incident reports, attendance feedback forms, etc.), statistics, reports/evaluations, and other important client information are reported and updated to the manager and relevant client records system. Acts as an advocate for clients by providing assistance/links to a variety of resource networks, services and programs in the community as a way of improving their overall health and well-being.
