Age UK Cornwall & The Isles of Scilly is seeking an additional team member to join its Gateway Outreach team, to support adults through person-centred, strengths-based conversations focused on “What Matters” to the individual.
This role will involve leading, supporting and guiding the team to use the My CAW (Measuring My Concerns and Wellbeing) assessments, co-producing with the client, Personalised Support Plans that promote self-management of health and wellbeing, early intervention, and prevention. The service involves short-term, timely support, empowering individuals to access appropriate statutory and voluntary sector services and regain control over their lives. Your leadership will ensure a safe, effective and efficient service which meets the needs of individuals.
This role would suit someone with strong interpersonal and networking skills who can empathise and provide support to their team in a way that inspires trust and confidence.
