Description
We are seeking a reliable CNA or HHA with experience in 1:1 patient care to provide compassionate, personalized support to clients in their homes. This role involves assisting with daily living activities, monitoring health conditions, and ensuring client safety and well-being. You will work under the supervision of our clinical team and office staff, and may assist with light housekeeping and administrative tasks.
Key Responsibilities:
• Personal Care: Assist clients with daily activities such as bathing, dressing, grooming, and toileting, ensuring they maintain good hygiene and comfort.
• Mobility Assistance: Help clients move around their home, including transfers from bed to wheelchair, walking, and positioning.
• Medication Management: Remind clients to take prescribed medications and monitor for side effects or changes in health, reporting any concerns to the supervising nurse.
• Health Monitoring: Observe and report changes in the client’s physical or mental health, including vital signs like blood pressure and temperature.
• Household Assistance: Perform light housekeeping duties, including cleaning, laundry, and meal preparation, to maintain a safe and sanitary environment.
• Companionship: Provide emotional support and companionship to clients, helping them stay engaged and active in their daily lives.
• Safety: Ensure the home environment is safe by identifying and addressing potential hazards and assisting with any required medical equipment.
• Documentation: Maintain accurate records of care provided, changes in the client’s condition, and any incidents during care.
• Communication: Communicate effectively with clients, family members, and healthcare professionals to coordinate care and address any concerns.

Pay: $15.00 - $18.00 per hour
