Overview
Otterbein has an employment opportunity for a Care Coordination Specialist. This role with collaborate with the Admissions Specialists, HealthCare Navigators (marketing) and community staff to ensure residents and families receive outstanding customer service upon arrival and throughout their stay.
The Care Coordination Specialist will:
Lead tours for prospective residents and families
Develop and maintain a list of potential new long-stay referrals
Oversee all case management and discharge planning of residents during their skilled stay
Manages appeals processes with insurance companies
Supports business office functions

Primary Duties and Responsibilities
Provide exceptional customer service to all incoming residents and families and ongoing throughout the resident’s stay
Collaborate with Central Hub Intake Specialists, Community team, and Navigators to facilitate smooth / successful Admissions and communicate current needs or barriers related to census / admissions
Develop and maintain strong relationships with all internal and external customers to yield an increased share of satisfied patient referrals from targeted referral partners
Act as Community representative through the internal admission process
Facilitate flow of information regarding new admissions and census changes between the Community and Hub
Collaborate with Central Intake Admission Specialist to assist in all Admission Agreements within 72 hours / 3 business days of any new admission or re-admission
Responsible for verifying and completing daily census reporting
Provide tours and information, as needed to prospective residents and rehab guests and their families
Ensure “Room readiness” for admissions – manage First Impressions
Responsible for collecting initial payment at the Community from new or transitioning residents / families
Deliver welcome information / materials for new residents
Order supplies and specialized equipment as needed to facilitate new admissions and ensure returns are made
Facilitate the gathering of report information for long-term new Admissions
Ensure completion of Patient Inventory assessment by nursing staff
Monitor Community trackers / reports and complete as needed to collaborate with Community admissions specialist & community partners – HENS, PAS/RR, Admissions Agreements, etc.
Provide case management for skilled patients
Oversee NOMNC and DENC process, delivering to patient and obtaining necessary signatures
Manage insurance appeals
Coordinate and support discharge planning
Participate in utilization review, leading process in conjunction with rehab ADON
Track PDPM requirements and related documentation
Obtain level of care determinations from the appropriate community agency.
Provide resident bed hold letters as needed
Obtain necessary authorizations for long-term care
Participates in the Otterbein Shepherd program and necessary follow-up to address concerns
All other duties as assigned
Qualifications
Education: Graduate of approved nursing program
Licensure: Valid Ohio Licensed Practical Nurse (LPN) license, CPR
Valid Drivers License
Experience: Minimum of three (3) years’ experience in customer service or sales role. Experience in healthcare or long-term care is highly desirable.
