Welcome, Thank you for your interest in becoming a provider for A Cognitive Connection, LLC (ACC) / H.E.R.O.E.S. The following is a questionnaire and employment/contractor inquiry, and authorization for background inquiry. Please complete it entirely so we can begin your application process. You will only be considered for a possible position when all forms are filled out completely. Completion of this questionnaire and background check does not guarantee, imply, or contract you as a counselor or direct care provider. If you are chosen as a prospective provider, you will be notified by the administration to start the orientation process. Thank you for considering us, Carol and Jason Van Ness  (Co-Executive Directors) Responsibilities: • Provide 1:1 implementation of the individual’s Behavioral Support Plan under the supervision and oversight of a Behavioral Consultant. • Deliver acute, short-term behavioral interventions to address identified challenging behaviors that pose a risk to the individual’s health and safety and/or the safety of others. Maintain focus on immediate stabilization, safety, and risk reduction during behavioral episodes. • Support individuals transitioning from institutional settings or at risk of institutional placement. • Utilize approved crisis intervention strategies, with Safety Care and/or TCI approaches recommended at the time of enrollment. • Provide Behavioral Line Services in the individual’s home and/or in community-based settings, including day programs, by supporting the individual and assisting program staff in maintaining safety and implementing behavioral strategies.
