We are seeking a highly skilled and compassionate Registered Nurse (RN)in theAlbuquerque/ Santa Fe, NM area to join our team and make a meaningful impact on the lives of our patients.   As a Resource Nurse, you will travel 2–3 days per week to support a variety of clinical responsibilities. This role is responsible for completing assessments for new, transferred, and current patients, as well as reviewing, developing, and coordinating individualized plans of care. You will provide education to patients, their families, and home health aides regarding medical conditions, disease progression, and care needs. The Resource Nurse will also support the onboarding and training of contracted caregivers. In addition, this position provides clinical support to the Director of Nursing, Compliance Manager, and the broader care team. Strong collaboration with patients, families, skilled and non-skilled caregivers, and interdisciplinary team members is essential to ensure effective care planning, service utilization, and overall patient outcomes. This role also includes connecting patients with appropriate community resources and recommendations.   This is a full-time, Monday-Friday, 8am -5pm. Some evenings may be required.    Base salary is $80-$100,000 with potential to make up to $14,000 in annual bonuses based on performance metrics.   #INDNM/ABQ
