Description
Our client seeking a compassionate and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing primary care team. The ideal candidate will provide high-quality, patient-centered care in a collaborative environment focused on prevention, chronic disease management, and improving patient outcomes. Essential Duties and Responsibilities Conduct comprehensive patient assessments, examinations, and evaluations. Diagnose and treat acute and chronic medical conditions. Order, review, and interpret laboratory tests, imaging studies, and other diagnostic procedures. Develop and implement individualized treatment plans. Prescribe medications and therapies in accordance with applicable regulations and organizational policies. Provide preventive care services, annual wellness visits, and patient education. Manage chronic conditions including hypertension, diabetes, obesity, COPD, asthma, and other common primary care diagnoses. Review and address laboratory results, imaging reports, patient messages, and clinical tasks in a timely manner. Maintain  accurate, complete, and timely medical documentation. Coordinate care with specialists, hospitals, and ancillary healthcare providers. Participate in quality improvement initiatives and organizational programs designed to improve patient outcomes. Maintain compliance with all federal, state, payer, and organizational requirements. Requirements    Required Current Michigan Nurse Practitioner or Physician Assistant license, or eligibility to obtain prior to employment. Current DEA registration or eligibility to obtain. Current BLS certification. Strong communication, interpersonal, and clinical decision-making skills. Commitment to providing exceptional patient-centered care. Preferred Experience in Primary Care, Family Medicine, Internal Medicine, Urgent Care, or Geriatric Medicine. Experience utilizing electronic medical records.
