This Per Diem Physical Therapist role offers flexibility, autonomy, and the opportunity to provide high-quality rehab and community-based care directly to patients in their homes. As a Home Health Physical Therapist , you will work with senior care populations , including geriatric patients recovering from illness, injury, or surgery. You’ll deliver skilled therapy services in home-based, assisted living, and community care settings , helping patients regain independence and improve their quality of life. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients .
