Akicita Federal is hiring Licensed Practical Nurses (LPNs) to deliver direct patient care in support of the VA Primary Care team at the Dayton VA Medical Center in Dayton, OH. In this role, you will work alongside an interdisciplinary PACT team to provide coordinated, veteran-centered primary care in a fast-paced clinical environment. This is an opportunity to make a meaningful impact on the health and well-being of those who have served our country. - Obtain and document patient histories, perform clinical assessments, and contribute to individualized care plans within the electronic medical record. - Administer prescribed medications, verify medication orders across shifts, and document patient responses accurately. - Monitor and document fluid intake and output each shift; perform routine skin assessments and execute measures to prevent pressure ulcers. - Communicate changes in patient condition promptly to the charge nurse or supervising physician and assist in revising care plans as needed. - Operate clinical equipment including EKG machines, cardiac monitors, and defibrillators; deliver central line and ventilator care in accordance with facility policies. - Educate patients and families on medications, therapies, and diagnostic procedures, and document their demonstrated understanding. - Recognize and respond to acute changes in patient status, initiate appropriate emergency interventions, and participate in multidisciplinary discharge planning.
