Description
We are a mission-driven organization providing services to individuals with developmental disabilities under OPWDD (Office for People With Developmental Disabilities). We are seeking a dedicated and organized Coordinator to join our team in New City, NY. The Coordinator will serve as a vital link between individuals, families, providers, and regulatory agencies. This role involves managing service plans, coordinating care, and ensuring compliance with OPWDD regulations. The ideal candidate is detail-oriented, compassionate, and committed to promoting dignity, independence, and community inclusion for those we support. Key Duties Include: Coordinating OPWDD-funded services including day programs, residential, respite, and clinical care Developing and monitoring Person-Centered Service Plans (PCSPs) Conducting regular follow-ups, home or site visits, and annual reviews Maintaining accurate records and ensuring documentation compliance with OPWDD standards Facilitating communication among stakeholders, including families, providers, and OPWDD regional offices Advocating for individuals and ensuring access to appropriate services and supports Requirements Ability to manage multiple cases while maintaining attention to detail Strong organizational and time-management skills Excellent written and verbal communication Ability to work independently and as part of an interdisciplinary team Must have a valid NYS driver’s license and reliable transportation Availability for in-person work based in New City, NY Qualifications Associate’s degree required; Bachelor’s degree in human services, psychology, social work, or a related field strongly preferred 1–2 years of experience working with individuals with developmental disabilities, preferably within the OPWDD system Familiarity with Person-Centered Planning and Medicaid Waiver services Experience using electronic health records and Microsoft Office CPR/First Aid certification preferred Bilingual candidates are encouraged to apply
