AOD Outreach & Community Engagement Program Part Time (0.8 FTE) - Fixed Term until 30 June 2027 About the role North Richmond Community Health (NRCH) is seeking an experienced and compassionate Complex Care Coordinator to join our AOD Outreach & Community Engagement (OAE) Program. This is an opportunity to work alongside people who use drugs, providing holistic, trauma-informed care coordination and assertive outreach that supports people to achieve their own goals. Working as part of a multidisciplinary team, you'll build trusting relationships, deliver evidence-based harm reduction interventions, and help improve health and wellbeing for some of our community's most marginalised members. This role is community-based and combines complex care coordination with assertive outreach, clinical assessment, community engagement and harm reduction education throughout the North Richmond precinct. Deliver evidence-based, trauma-informed harm reduction services to people who use drugs. Undertake assertive outreach and engage with people experiencing complex challenges, including AOD use, homelessness, and physical and mental health concerns. Complete holistic assessments, develop care plans and coordinate support within a shared caseload framework. Conduct clinical assessments, identifying when escalation to medical services is required and facilitating appropriate referrals. Collaborate with the Medically Supervised Injecting Room (MSIR), multidisciplinary colleagues and external services to deliver integrated, person-centred care. Build positive relationships with community members and contribute to overdose response, harm reduction education and improved public amenity across North Richmond. Regular evening and weekend work is an essential requirement of this role. About you You are a Registered Nurse who is passionate about harm reduction, social justice and delivering person-centred care. You thrive working autonomously in the community while collaborating closely with a multidisciplinary team and external services. You will bring: Current Division 1 Registration with AHPRA Qualifications in Alcohol and Other Drugs (AOD) (or willingness/experience as outlined in the PD) Experience supporting people who use drugs and people experiencing complex health and social needs Strong clinical assessment and care coordination skills A commitment to trauma-informed, culturally safe and person-centred practice Excellent communication and relationship-building skills Current First Aid Certificate (or willingness to obtain) Experience in outreach, community engagement, mental health and/or complex case management will be highly regarded.
